Issuance of CSD Card

Procedure for obtaining a new CSD Card for the family in case of death of original pensioner in brief as the operation of old card will discontinue. The following documents are required for a new Card-

1. Original CSD Card and death certificate of the deceased pensioner.
2. Self attested copy of the new PPO with original copy of new PPO for verification.
3. Aadhar Card in original for verification with self attested copy of the Aadhar card of the family pensioner.
4. PAN card of the family pensioner for verification.
5. Two passport size photographs of the family pensioner.
6. Obtain the required form, from nearby CSD, fill the same and submit the same to the CSD with the above documents.
7. The new CSD card will be given to the Pensioner within one month.
8. The entitlement of the family pensioner will be the same as per the original CSD Card.
