In case of death of pensioner holding CGHS Card

 

In case of death of pensioner holding CGHS Card, the dependents can continue using CGHS benifits by converting the card to a family pensioner card.(Source FAQ no.20 My CGHS 2.) FOR Conversion following actions are required.-

  1. Visit AD Office with copy of the death certificate, Aadhar Card and the CGHS Card of the original pensioner.
  2. CGHS Card, Aadhar Card and copy of the PPO of the family pensioner.
  3. Fill the prescribed form and submit the same. Further action will be taken by the office and same card of the family will change as primary card.

Note 1. This can be done online by the spouse and card of the original pensioner can be surrender in the parent dispensary.(Source as per actual case)
